Specialist Accreditation

Mount Hospital welcomes Health Practitioners to apply for credentialing to our facility by completing the online application form. Alternatively, if you would like to discuss applying for credentialing please call (08) 9327 1100 during business hours. All applications are reviewed by our General Manager for temporary approval subject to receipt of all required information. For full approval, this will be completed by the Credentialing Committee at the next scheduled meeting. We will require you to upload the following documentation along with your application form:

  • Current Curriculum Vitae
  • Copy of current AHPRA Registration
  • Contact details for 2 referees
  • Copy of current Professional Indemnity Insurance certificate of currency showing:
    • Period of insurance
    • Amount of cover
    • Category of insurance (Please note we do not accept copies of tax invoices, renewal notices or receipt).
  • National Police Check (completed in the last 3 years)
  • Working with Children Check
  • Hand Hygiene Certificate (completed in the last 12 months)
  • Aseptic Technique Certificate (completed in the last 12 months)
  • Basic Life Support Certificate (completed in the last 3 years)
  • Open disclosure training certificate
  • Immunisation evidence (Hep B, Hep C, MMR, Varicella, Pertussis, Tetanus, Diphtheria, TB, MRSA, COVID-19, Influenza)

If applicable:

  • Copy of Radiation Licence (this is legally required if you intend to operate x-ray equipment)
  • Copy of GESA certification
